Burlington, Ia- Des Moines County Public Health will be offering a COVID-19 vaccination clinic for Burlington High School students.
The clinic will take place at BHS on April 28, 2021.
BHS students who are 16 years of age on or before April 28th are eligible to receive the vaccine at this event.
This is a two-dose vaccine and the 2nd dose will be given at BHS on May 19.
A parent/guardian of the student must complete a form to be returned to the Burlington School Office by Friday, April 23rd in order for the student to participate in the clinic.
