A nursing home administrator in West Point has been sanctioned for quality-of-care issues at a Lee County nursing home she managed.

The Iowa Board of Nursing Home Administrators has filed charges against Miriam Johnson, accusing her of professional negligence tied to an alleged failure to use appropriate care and judgment.

Johnson was serving as administrator of the 46-bed West Point Care Center in January 2024 when the facility was cited for several violations. Those included failing to report suspected abuse and not properly investigating abuse allegations.

State inspection records show multiple employees raised concerns about a certified nursing assistant, describing the worker as verbally abusive toward residents with dementia.

Alongside the filing of charges, the board reached a settlement agreement requiring Johnson to complete 10 hours of education focused on resident abuse prevention and quality assurance practices.

Despite the cited issues, the West Point Care Center currently holds an overall five-star rating under the Centers for Medicare and Medicaid Services’ quality system.
