The former administrator of Aspire of Donnellson has been disciplined by the Iowa Board of Nursing Home Administrators following widespread quality-of-care issues at the Lee County nursing home. The board found Janelle Case was the facility’s administrator until January 2024, when inspectors cited the home for numerous violations involving staffing, medication errors, infection control, training and overall patient care after investigating multiple complaints.
Under a settlement agreement, Case admitted no wrongdoing but agreed to complete additional training in quality assurance and leadership. Her nursing home administrator’s license will remain on probation for one year, during which she will be supervised by another licensed administrator who will submit quarterly reports to the state board.
State records show Aspire of Donnellson was cited for even more violations later in 2024 after a fire forced residents to evacuate. The facility’s license was revoked, and it subsequently closed. Case is now the administrator at the Bloomfield Care Center in Davis County.
