The Burlington Hy-Vee Health Exemplar Care clinic will be located at 1000 N. Roosevelt Ave. The Hawkeye reports that while no opening date has been set, the ribbon cutting will include an open house with facility tours and the chance to meet the care team.
“Hy-Vee Health Exemplar Care is a membership-based primary care model, often referred to as direct primary care designed to simplify healthcare access while making costs more predictable for members. Once the clinic opens, people will be able to enroll online or in person, and begin scheduling care right away. Memberships can be used independently or alongside traditional insurance.
